A. She has probably an allergic dermatitis. Most likely it is related to fleas.

On top of the allergic disease she may have a bacterial or another (yeast for example) infection of the skin which is causing the persistent ''bad smell''.

I would suggest to take her to your veterinarian for a check up and appropriate treatment. You veterinarian can recommend a medicated shampoo and prescribe a good flea treatment for her. She may require antibiotics as well.
