A. These are signs of severe disease in a 15 year old cat - most commonly at ths age: kidney failure or a tumor. Both with bad prognosis and very limited treatment options.

I would advise taking her to the vet for a check and a blood test, possibly an ultrasound, then decide what's the best thing to do.
