Q. Enlarged heart puppy. I believe it is a congenital defeat. He is 5 months old and a shih tzu mix

A. It might be a bit too much to take on board. His life is probably going to be shorter because of this problem , which perhaps could be corrected by surgery ? If possible to operate it s going to be very expensive. There are lots of things to consider. In a way if you adopt him you are going to give him the chance of his life but you need to think carefully.
