A. The dosage of insulin would depend on his weight so it is difficult for me to say if the dosage it is appropriate. Other concomitant diseases can create a insulin resistance (so the insulin won't work) and ideally they should be ruled out if insulin is not effective. The efficacy of the insulin would depend as well on what type of insulin you are injecting, so we would definitively need more informations to be able to be more helpful with the advices.
Aural hematoma is usually caused by an underlying cause (like inflammation of the ear canal, which cause discomfort and scratching/shaking of the head). Otitis should be resolved.
