A. The hip is probably arthritis, a condition called "Hip Dysplasia", very common in big breeds and usually needs long term anti inflammatory treatment (special dog anti inflammatories only) and joint supplements such as Flexadin.
The skin condition is probably caused by an allergic reaction. first of all make sure you flea your dog regularly every month with a high quality spot on (ex. Advantage) as fleas are by far the most common reason for these reactions.
If the fleas are not the problem, it could be an allergic reaction to food or an environmental factor. this is more complicated to treat and will sometimes require medications such as Steroids, anti histamines or cyclosporin in order to stop the allergy.

A. You should take your dog to a vet for an exam, possible allergy testing and also assessment of the hips. Your vet can prescribe antiinflammatories for the hip issue, which in turn take the pain, and also advise on the skin issues. He may want to do radiographs and skin scrapings/biopsies.
